Happy New Year! We want to bring your attention to some of our office policies as we enter into 2025:​

• Please be patient when coming in for the first time in 2025.

• Please be prepared to update your patient information forms and provide us with a copy of your insurance card.

• If applicable, we will need a copy of your Medicare card (the red, white, and blue card), even if you participate in a Medicare Advantage plan.

• Please take note of the updated terms in our “No Show” policy.

• Please remember that for all referrals, we will need you to provide the necessary information with at least 48-hour notice.

• Unless it is an emergency, please allow us at least a week to get back to you for authorization for all medications and procedures.
